SERVICES
1. Issuance of Initial National Identity Card
• The applicant must be 15 years of age to apply for an NIC initially.
• A fine of Rs. 2,500 will be imposed on those applying after 15 years of age.
• Required documents (to be submitted via the Grama Niladhari Officer):
o Completed application form
o Original birth certificate
o Photograph meeting ICAO standards, taken by a registered company approved by the Department of Registration of Persons

2. Modification of National Identity Card
• Required documents:
o Old NIC
o Photograph meeting ICAO standards, taken by a registered company approved by the Department of Registration of Persons
o Written evidence justifying modification (e.g., if NIC is damaged or destroyed)
• One-day service fee: Rs. 2,500

3. Reissuance of Lost National Identity Card
• Required documents:
o Completed application form
o Police report
o Original birth certificate
o Photograph meeting ICAO standards, taken by a registered company approved by the Department of Registration of Persons
o Written proof of prior NIC use (e.g., Passport, Passbook, Driving Licence)
• One-day service fee: Rs. 3,000

SUBMISSION PROCESS
• All required documents must be certified by the Grama Niladhari Officer.
• Certified documents must be submitted to the Department of Registration of Persons via the District Secretary’s Office for one-day service.
