Registration of Expired Births under Act No. 24

Requirements:

1. Registration of births in hospitals or estates.
2. Birth certificates of the mother and father.
3. Marriage certificate of the mother and father.
4. Copies of NIC (National Identity Card) of the mother and father.
5. Affidavit.
6. Grama Niladhari certificate.
7. Health development statement.

Certified copies of all documents.

Publication Fee: Rs. 60.00

Amendment of Names – Under the 27th Act (For Children Under 7 Years)

Required Documents:

1. Applicable birth certificate
2. Affidavit
3. Consent letters from both father and mother
4. Marriage certificate of parents
5. Grama Niladhari certificate
   (Originals of all certificates are required)

Fee: Rs. 60 for publication

Process:
To modify the name in the birth certificate, Form B-9 (27th Amendment) must be completed and submitted along with the above documentation.

Amendment of Names – Under the 27th Act (For Ages 7 to 21 Years)

Required Documents:

1. Copies of birth certificate
2. Copies of parents’ marriage certificate
3. Copies of admission letter (school)
4. Three statements and three copies of documents that have been used for more than one year with the full name intended to be changed
5. Affidavit requesting the name change
6. Letter from Grama Niladhari regarding the name change
7. If the applicant is the mother, a letter of consent from the father
8. If the applicant is the father, a letter of consent from the mother
9. Newspaper notice
10. NIC and a copy of NIC

Fee: Rs. 60 for publication

Name Change Under the 21st Act (For Individuals Above 21 Years)

Required Documents:

1. Applicable birth certificate
2. Documents proving usage of the existing name for more than one year
3. Newspaper notice
4. Bank passbook
5. Water bill or electricity bill
6. Marriage certificate (if applicable)
7. Birth certificates of children (if applicable)
8. Documents to prove use of any other names
9. Grama Niladhari residential certificate
   (Originals of all documents are required)

Fee: Rs. 60 for publication

Modification of Father's Information Under the 27th Act (Sections 4, 6, 7)

Required Documents:

1. Applicable birth certificate
2. Birth certificate of father
3. Copy of father's NIC
4. Grama Niladhari certificate in father's name
5. Affidavit
6. Marriage certificate of parents
   (Originals of all documents are required)

Fee: Rs. 60 for publication

Modification of Mother's Information Under the 52nd Act (Sections 1, 3, 5)

Required Documents:

1. Applicable birth certificate
2. Birth certificate of father
3. Hospital birth record
4. Copy of mother's NIC
5. Grama Niladhari certificate in mother's name
6. Affidavit
   (Originals of all documents are required)

Fee: Rs. 60 for publication

Registration of Expired Death Under the 36th Act

Required Documents:

1. Statement of estate / hospital death report / Grama Niladhari report
2. Receipts of funeral expenses
3. Affidavit from a close relative
4. Grama Niladhari certificate
5. Documents proving religious rites or ceremonies were performed
6. Evidence of receiving death benefits

Fee: Rs. 60 for publication

   (Including date of birth and payment history of two years)

   (Including date of marriage and check history of two years)

   (Including registration number and date of death)

   (Including date of death and check history of two years)

7. Amendment of Names under the 27th Act

   For Children Under 7 Years

   Requirements:

   1. Applicable birth certificate.
   2. Affidavit.
   3. Consent letters from both father and mother.
   4. Marriage certificate of parents.
   5. Grama Niladhari certificate.

   Note: Original copies of all documents are required.
   Publication Fee: Rs. 60.

   For Individuals Aged Between 7 and 21 Years

   Form Required: 27th Amendment Form B-9 (must be completed and submitted).

   Documentation Required:

   1. Copies of birth certificate.
   2. Copies of parents’ marriage certificate.
   3. Copies of school admission letter.
   4. Three (3) statements and three (3) copies of documents that have been used for more than one year with the full name to be changed.
   5. Affidavit requesting name change.
   6. Letter from Grama Niladhari regarding the name change.
