Tasks

  • Issuing of permits for felling timber, transporting timber, mortgages, excise duties, and transporting animals.
  • Issuing recommendations related to timber business registration.
  • Providing recommendations for firearms and explosives licenses.
  • Issuing recommendations related to the issuance of conductor licenses

 

Felling of wood After completing the application and obtaining the recommendation from the Grama Niladhari, the following documents should be submitted:

  • Certified copy of the deed of the land on which the relevant tree is located
  • Plan and sketch of the land.
  • Approved building plan as appropriate

 

Transport permitsPermits are issued for the transportation of legally felled timber.The following documents must be submitted with the completed application form.

  • Certified copy of the deed of the land from which the timber was obtained
  • If wood was purchased, the relevant receipt
  • Cutting permit for timber that requires a permit to cut
  • List of timber
  • Plan of the land with the location of the trees marked.
  • When timber transportation applications are recorded in the approval submission register and permits are issued based on the recommendation of the Assistant Divisional Secretary and the approval of the Divisional Secretary.
  • The license fee is Rs. 200.00 (18% VAT Rs. 36.00)
  • The permit must be written and signed by the Assistant Divisional Secretary or Divisional Secretary before the scheduled date of transportation.
  • The permit and timber certificate must be issued to the Grama Niladhari on the day of transportation.

Registering a timber business

Here is done to facilitating the legal operation of timber businesses.

Required Qualifications

  • Being someone who is looking to start a timber business
  • Location subject to the relevant distance restrictions as per Circular 1997/02 dated 17.11.1997
  • Not having been involved in any crime within the past five years
  • The applicant's request letter along with the application issued by the District Forest Officer and the following documents should be submitted
  • Documents confirming ownership of the land
  • Environmental Protection License
  • Grama Niladhari Report

Issuance of mortgage licenses Here, legal permission is granted to operate a pawnshop business. The first time a pawnshop license is issued is by the Department of Revenue in Western Provinceequired Qualifications

  • Being a citizen of Sri Lanka
  • Not having been convicted of a crime before the law
  • The business must be located within the relevant Divisional Secretariat Division.

Required documents

  • Application submitted in the prescribed format
  • Police report on the location
  • Grama Niladhari Report
  • Business registration certificate
  • Fire insurance coverage Theft insurance coverage
  • Affidavit
  • Records confirming ownership of the business premises

The above documents should be submitted to the Western Provincial Revenue Department.

Annual renewal of mortgage licenses

The mortgage license is validated annually.

Required Qualifications

  • Being a licensed pawn shop
  • Having a valid license for the previous year

Required documents

  • Application form
  • Grama Niladhari Report
  • Police Report
  • Financial report of the last year
  • Copy of the mortgage license from the previous year
  • Fire insurance coverage Theft insurance coverage
  • Affidavit
  • Letter from the Provincial Revenue Commissioner requesting the issuance of the license

Issuance of a new liquor shop license

This is where legal permission to operate a liquor store is granted.

Required Qualifications

  • The applicant must be a person fit to hold the license
  • There is no public opposition to the issuance of the license
  • The location of the place is inconsistent with the relevant guidelines

Required documents

  • Letter issued by the Commissioner General of Excise requesting the issuance of the excise license
  • Excise license obtained for the previous year
  • The above documents should be submitted to the Divisional Secretary for approval.
  • After approval, the relevant license fee must be charged.
  • The period, license fee, and boundary details must be specified in the license.
  • The permit must be submitted to the Divisional Secretary for signature.
  • The license must be issued after the licensee signs the copy agreement recorded in the license issuance document.

Issuance of permits for the transport of animals

 

Legal permission is granted here for the transportation of animals.

Required Qualifications

  • Confirming that the animal is healthy
  • Being an animal/animals that confirms ownership of the animal
  • If a cow is branded
  • Letter of consent from the owner with the recommendation of the Grama Niladhari

Required Documents

  • Request letter of the applicant
  • Recommendation of Grama Niladhari
  • Veterinary report
  • Documents confirming ownership of the animal
  • Report of the vehicle being transported

 

Charges

  • Transportation fees are charged at Rs. 50.00 per cow.

Issuance of firearms licenses

 

Recommendations are made here for the issuance of firearms licenses to allow the use of firearms to prevent crop damage. The license is issued by the District Secretariat.

Required Qualifications

  • The applicant must have cultivated land of more than 5 acres.
  • Damage to crops by animals
  • Being under 65 years of age
  • Being physically fit to use a firearm

  • Request letter and relevant application sent by the District Secretary seeking recommendations for the issuance of a firearms license
  • Deed of the relevant land/lands
  • Grama Niladhari’s Report
  • The following reports on lands and crops
  • Report of the Small Tea Estates Development Authority
  • Rubber Control Department
  • Agricultural Service Center
  • Report of the Coconut Cultivation Board for Coconut Cultivation

Issuance of explosives licenses

The relevant recommendation for granting legal permission for the use of explosives is made here. The issuance of the license is done by the District Secretariat.

Required Qualifications

  • Being an industrialist who uses explosives
  • Having technical ability
  • Confirming the need for explosives

  • Application issued by the Explosives Controller with a request letter
  • Grama Niladhari’s report
  • Environmental Report / Environmental Protection Permit
  • Police report
  • State Officer Report as applicable
  • Valid mining license if mining industry

Civil Registration

Registration of Expired Births under Act No. 24

Requirements:

  1. Registration of births in hospitals or estates.
  2. Birth certificates of the mother and father.
  3. Marriage certificate of the mother and father.
  4. Copies of NIC (National Identity Card) of the mother and father.
  5. Affidavit.
  6. Grama Niladhari certificate.
  7. Health development statement.

Certified copies of all documents.

Publication Fee: Rs. 60.00

Amendment of Names – Under the 27th Act (For Children Under 7 Years)

Required Documents:

  1. Applicable birth certificate
  2. Affidavit
  3. Consent letters from both father and mother
  4. Marriage certificate of parents
  5. Grama Niladhari certificate
    (Originals of all certificates are required)

Fee: Rs. 60 for publication

Process:
To modify the name in the birth certificate, Form B-9 (27th Amendment) must be completed and submitted along with the above documentation.

Amendment of Names – Under the 27th Act (For Ages 7 to 21 Years)

Required Documents:

  1. Copies of birth certificate
  2. Copies of parents’ marriage certificate
  3. Copies of admission letter (school)
  4. Three statements and three copies of documents that have been used for more than one year with the full name intended to be changed
  5. Affidavit requesting the name change
  6. Letter from Grama Niladhari regarding the name change
  7. If the applicant is the mother, a letter of consent from the father
  8. If the applicant is the father, a letter of consent from the mother
  9. Newspaper notice
  10. NIC and a copy of NIC

Name Change Under the 21st Act (For Individuals Above 21 Years)

  2. Documents proving usage of the existing name for more than one year
